Why Paramus patients travel for vestibular care specifically

Bergen County is not short of physical therapy. What is genuinely scarce is a clinic that treats the vestibular system as a specialty rather than a line item — and that is what brings people west from Paramus, Ridgewood Avenue, the Route 17 corridor and the neighborhoods around Bergen Town Center.

Dizziness is the problem most often mishandled in general orthopedic clinics. It gets labelled “inner ear” and treated with generic balance exercises, when the actual job is to work out which system is failing — the crystals in the inner ear, the vestibular nerve, a migraine mechanism, or a neck contribution — because each one has a different treatment and a very different timeline.

What we sort out at the first visit

BPPV — the positional kind

The room spins for under a minute when you roll over in bed, tip your head back, or look up. This is the most mechanically fixable form of vertigo there is, and it is treated with a repositioning maneuver rather than exercises.

Vestibular neuritis

A sudden, severe, constant spinning that arrives over hours and often follows a virus. Recovery depends on properly dosed compensation exercises — done wrong, people stay dizzy for months.

Vestibular migraine

Dizziness with light or motion sensitivity, sometimes with no headache at all. Frequently missed for years.

PPPD — persistent postural-perceptual dizziness

A constant rocking or swaying that is worse in busy visual environments — supermarkets, the mall, scrolling a phone. Common after an initial vertigo episode resolves.

Balance loss and fear of falling

Often no spinning at all, just unsteadiness that has quietly narrowed what you are willing to do.

Our own vestibular numbers

We tracked and published our results rather than quoting the literature. Across 30 consecutive vestibular patients, of the 28 vestibular patients who completed care, 23 of the 28 completed care in three visits or fewer. Among the 11 patients who completed care for BPPV, treatment took a median of two visits, and 9 of the 11 finished in three visits or fewer.

Note what that means practically: this is usually a short course of care, not a months-long program. If someone quotes you twelve visits for positional vertigo before examining you, ask why.

Getting here from Paramus

About 15 miles, roughly 25 to 30 minutes in normal traffic. From most of Paramus the run is Route 4 or Route 17 to Route 208 North, then across to Hamburg Turnpike; from the north end of town Route 208 is usually the cleaner run.
