What Causes Elbow Pain and How Can Physical Therapy Help?

Elbow pain most commonly comes from overuse injuries like tennis elbow or golfer's elbow, nerve compression such as cubital tunnel syndrome, or recovery after a fracture. Physical therapy treats the root cause of elbow pain through hands-on manual therapy, targeted strengthening, and activity modification. Most elbow conditions respond very well to conservative treatment without surgery.

At Spectrum Therapeutics in Wayne, NJ, Dr. Rob Letizia treats elbow injuries with one-on-one care every visit. With over 25 years of experience in orthopedic manual therapy, he sees these conditions regularly in patients from Wayne, Pompton Lakes, Totowa, and across Passaic County.

What Is Tennis Elbow and How Do You Fix It?

Tennis elbow, or lateral epicondylitis, is pain on the outside of your elbow caused by overuse of the wrist extensor tendons. Despite the name, most people who get it have never picked up a racket. It's common in people who type, use tools, cook, or do repetitive gripping activities.

How Physical Therapy Treats Tennis Elbow

Dr. Rob uses a combination of manual therapy, eccentric strengthening exercises, and activity modification to treat tennis elbow. Eccentric loading, where the muscle lengthens under tension, has strong research behind it for tendon healing. Manual therapy to the forearm muscles and elbow joint helps reduce pain and restore normal tissue mobility.

For cases that have been lingering for months without improvement, shockwave therapy is an effective option. It works by stimulating the body's own healing response in damaged tendon tissue and is available at our Wayne, NJ clinic.

What Is Golfer's Elbow?

Golfer's elbow, or medial epicondylitis, causes pain on the inside of the elbow. It affects the wrist flexor tendons and is common in people who do a lot of gripping, throwing, or lifting. The treatment approach is similar to tennis elbow but targets the opposite muscle group with specific manual therapy and eccentric strengthening protocols.

Can Physical Therapy Help Cubital Tunnel Syndrome?

Yes. Cubital tunnel syndrome happens when the ulnar nerve gets compressed or irritated at the inside of the elbow. You might feel numbness or tingling in your ring and pinky fingers, weakness in your grip, or aching pain along the inner elbow. Physical therapy for cubital tunnel includes:

  • Nerve gliding and nerve mobilization techniques
  • Manual therapy to release soft tissue restrictions around the nerve
  • Postural and ergonomic education to reduce nerve tension
  • Strengthening of the forearm and hand muscles
  • Splinting recommendations when appropriate

Catching it early makes a big difference. If conservative treatment starts before significant weakness develops, most people avoid surgery.

What About Elbow Rehab After a Fracture?

After an elbow fracture, whether treated surgically or in a cast, regaining full range of motion is the biggest challenge. The elbow joint stiffens quickly, and scar tissue can limit both bending and straightening. Dr. Rob uses joint mobilizations, soft tissue work, and progressive stretching to restore movement. Strengthening comes next, gradually rebuilding the muscles around the elbow and forearm so you can return to normal activities.

Is This You?

If you are experiencing any of these symptoms, your elbow needs professional evaluation:

  • Burning pain on the outside of the elbow (Tennis Elbow)
  • Sharp pain when lifting a coffee cup or gallon of milk
  • Numbness or tingling in the ring and pinky fingers
  • Ache on the inside of the elbow (Golfer's Elbow)
  • Weakness in grip strength or dropping objects

What is Elbow Physical Therapy?

Elbow Rehabilitation focuses on the tendons and nerves that cross the elbow joint. Most elbow pain is repetitive strain (tendonitis). Unlike simple rest, physical therapy actively remodels the tendon tissue using specific loading protocols and releases nerve entrapments that cause numbness.

At Spectrum Therapeutics in Wayne, we treat the cause of the strain, which often starts at the shoulder or wrist.

Conditions We Treat

Tennis Elbow

Lateral Epicondylitis. Pain on the outside of the elbow. We use cross-friction massage and eccentric loading to heal the tendon.

Golfer's Elbow

Medial Epicondylitis. Pain on the inside of the elbow. Common in golfers, climbers, and construction workers.

Cubital Tunnel Syndrome

Compression of the Ulnar Nerve ('funny bone') causing numbness in the pinky. We use nerve gliding to free the entrapment.

Little League Elbow

Overuse injury in young throwing athletes. We focus on mechanics and rest protocols to protect the growth plate.

Olecranon Bursitis

Swelling at the tip of the elbow. We treat the inflammation and modify mechanics to prevent recurrence.

Post-Fracture Rehab

Stiffness after a cast or surgery. The elbow gets stiff very fast; expert mobilization is required to regain full extension.

Why Choose Physical Therapy First?

Elbow pain is often mechanical. Pills can't fix mechanics.

VS. Cortisone Shots

Cortisone provides temporary relief but studies show it can actually weaken tendon tissue and increase recurrence rates for Tennis Elbow.

VS. Surgery

Surgery for elbow tendonitis is rare and invasive. Over 90% of cases resolve with proper physical therapy, avoiding the risks of nerve damage or infection.

VS. Bracing Alone

A brace helps symptoms but causes muscle atrophy if used alone. We combine bracing with strengthening to ensure you don't become dependent on it.

What Our Patients Say

Common Questions About Elbow Therapy

What is the difference between Tennis Elbow and Golfer's Elbow?

Tennis Elbow (Lateral Epicondylitis) causes pain on the OUTSIDE of the elbow, usually from gripping or typing. Golfer's Elbow (Medial Epicondylitis) causes pain on the INSIDE of the elbow, often from lifting or flexion. We treat both by addressing the specific tendon load and mechanics.

Why do my fingers go numb?

Numbness in the ring and pinky fingers is often Cubital Tunnel Syndrome (ulnar nerve entrapment at the elbow). Numbness in the thumb and pointer finger is often Carpal Tunnel (wrist) or a pinched nerve in the neck. We perform differential diagnosis to treat the exact source.

Can physical therapy fix elbow tendonitis without shots?

Yes. Cortisone shots mask pain but can weaken tendons long-term. Physical therapy uses 'eccentric loading' protocols to realign the collagen fibers of the tendon, actually healing the tissue rather than just hiding the pain.

PT vs. Chiropractor: What's better for elbow pain?

Physical Therapists specialize in soft tissue and extremity joints. While chiropractic is great for the spine, PT is the gold standard for tendon rehab (Tennis/Golfer's Elbow) because it involves the strengthening required to rebuild the tendon's capacity.

Should I wear a brace for elbow pain?

A counterforce brace can provide temporary relief by changing the angle of pull on the tendon, but it is not a cure. You must strengthen the tendon to fix the problem permanently. Reliance on a brace can actually weaken the muscle over time.

How long does it take to fix Tennis Elbow?

Tendon tissue heals slower than muscle. Most patients feel relief within 3-4 sessions, but full remodeling of the tendon takes 6-12 weeks. Consistency with your home exercises is key to speed up recovery.

Do I need a referral?

No. New Jersey is a Direct Access state. You can schedule directly with Dr. Rob immediately without waiting for a physician's prescription.

Is dry needling used for elbow pain?

We focus on manual therapy and shockwave therapy (if applicable), which are often more effective for elbow tendinopathies. Dr. Rob will determine the best modality for your specific case during the evaluation.

Does insurance cover this?

Yes. We accept Medicare, BCBS, Aetna, Cigna, United, and most major plans. We will verify your exact coverage for you before you arrive.

Why does my elbow hurt when I wake up?

Morning stiffness is classic for tendonitis. It means the tendon tightened up overnight. If you have numbness in the morning, you might be sleeping with your elbow bent, compressing the ulnar nerve. We can advise on better sleeping positions.
