You have had a distal biceps tendon repaired at the elbow, you are in a splint or a hinged brace, and the questions start: how long is physical therapy, when does the brace come off, and when can you lift anything again? The honest answer is about six months, and the order matters more than the speed. Protecting the repair comes first, motion second, and loading the biceps itself comes last.
This guide lays out the week-by-week path from a published protocol, shows where protocols disagree, and names the rule we use to keep the repair safe. Your surgeon's protocol always comes first; this is the map that helps you read it.
How long is physical therapy after distal biceps repair?
About six months to return to full activity. In the Saint Louis University distal biceps repair protocol, light biceps curls start at week 16 and return to activity and sport-specific training begins at week 26. Mass General Brigham's protocol likewise puts unrestricted return to sport at 6+ months (Mass General Brigham distal biceps protocol, 2021).
That surprises people, because the arm often feels fine long before then. The tendon is reattached to bone, and the protocols are built around protecting that repair while it heals, which is why heavy biceps work comes so late.
Distal Biceps Repair Recovery Timeline: Week by Week
| Weeks after surgery | What you are doing | Still off-limits |
|---|---|---|
| Week 1 | Splint with the elbow at 90 degrees; wrist and hand gripping exercises | Lifting; active elbow bending; turning the palm up against effort |
| Weeks 2–6 | Hinged brace, motion opened in steps set by your surgeon; shoulder, shoulder-blade, wrist and grip work; triceps isometrics from weeks 5–6 | Lifting; active elbow flexion or supination |
| Weeks 6–12 | Brace off (week 6 to 8, by protocol); light triceps, wrist and shoulder weights from week 8, adding about 1 lb a week | Loading the biceps |
| Weeks 12–16 | Biceps isometrics from week 12; arm bike from weeks 10–12; stretching and motion | Biceps curls with weight |
| Weeks 16–26 | Light biceps isotonics (curls) from week 16; two-handed plyometrics at 16, one-handed at 20–22 | Full-effort lifting and sport |
| Week 26 onward | Return to activity and sport-specific training, once your surgeon clears you | — |

Weeks 0–2: what should I be doing right after distal biceps surgery?
In week 1 the elbow is held in a posterior splint at 90 degrees, and the only exercise is gripping and moving the wrist and hand (Saint Louis University distal biceps repair protocol). The goals are to control pain and swelling and protect the repair. Mass General Brigham's protocol also lists no weight bearing through the arm, no active elbow flexion or forearm supination until week 4, and no lifting until week 8 (Mass General Brigham distal biceps protocol, 2021).
What should I be doing 2 weeks after distal biceps surgery?
Around week 2 you move into a hinged elbow brace. Its range is set by your surgeon and opens up in stages. In the Saint Louis protocol the brace runs from 45 to 100 degrees at week 2, 30 to 115 at week 4 and 15 to 130 at week 6 (Saint Louis University distal biceps repair protocol). Exercise covers the shoulder, the shoulder blade, the wrist and grip, and still no active elbow bending or turning the palm up against effort.
In Mass General Brigham's version, elbow motion at weeks 2 and 3 is passive, moved for you, inside the brace. Active motion with gravity taken out of the movement starts at week 4. To move past this phase, you need full active elbow bending and forearm rotation against gravity, without the brace and without more pain or swelling (Mass General Brigham distal biceps protocol, 2021). Triceps isometrics, where you tighten the muscle without moving the elbow, begin at weeks 5–6 (Saint Louis University distal biceps repair protocol).
Weeks 6–12: when does the brace come off?
Between weeks 6 and 8. Mass General Brigham discharges the brace at week 6; Saint Louis University discontinues it at week 8 (Mass General Brigham distal biceps protocol, 2021; Saint Louis University distal biceps repair protocol). From week 8 the Saint Louis protocol starts light weights for the triceps, wrist and shoulder, adding about 1 lb a week, while the biceps itself is still not loaded (Saint Louis University distal biceps repair protocol). Mass General Brigham begins gradual weight bearing through the arm at week 8, with the elbow bent, progressing to a straight elbow by week 10 (Mass General Brigham distal biceps protocol, 2021).
Weeks 12–16: when can I start strengthening my biceps?
This is where protocols differ most. The Saint Louis protocol starts biceps isometrics at week 12 and light biceps curls at week 16 (Saint Louis University distal biceps repair protocol). Mass General Brigham's protocol starts submaximal isometrics for the elbow flexors at week 10 and progresses to resisted curls over weeks 10–12 (Mass General Brigham distal biceps protocol, 2021). Neither is wrong. The safe week depends on the repair your surgeon did, which is why their protocol overrides both.
Week 16 onward: how long after bicep tendon surgery can I work out?
With the operated arm, gradually and late. Light biceps isotonics start at week 16, two-handed plyometrics at week 16 and one-handed at weeks 20–22, with return to activity and sport-specific training from week 26 (Saint Louis University distal biceps repair protocol). Mass General Brigham's return-to-sport criteria include full, painless elbow and wrist motion, pain-free home exercise and physician clearance (Mass General Brigham distal biceps protocol, 2021). Before then, legs and cardio are usually fine: Mass General Brigham lists treadmill walking and running and a stationary bike from weeks 7–10.
The 16-Week Biceps Load Rule
Nothing heavy through the biceps until your surgeon clears it. In the conservative protocol that means isometrics at week 12 and light curls at week 16. Everything before that, including the motion work, the triceps and shoulder strengthening and the cardio, is built so you arrive at week 16 with a full-range elbow, a strong shoulder and a repair that has had time to heal. If your surgeon's protocol is faster, follow it. Never go faster on your own.
The 26-Week Distal Biceps Return Protocol
- Protect (weeks 0–2): splint, then hinged brace; grip, wrist and hand only.
- Move (weeks 2–6): brace range opened by your surgeon; shoulder and shoulder-blade work; triceps isometrics from weeks 5–6.
- Build around it (weeks 6–12): brace off at 6–8; light triceps, wrist and shoulder weights from week 8.
- Wake the biceps (weeks 12–16): biceps isometrics from week 12.
- Load it (weeks 16–26): light curls, then plyometrics.
- Return (week 26 onward): sport and full activity, on your surgeon's clearance.

Red flags after distal biceps repair
Contact your surgeon promptly for any of these (Mass General Brigham distal biceps protocol, 2021):
- a fever;
- numbness or tingling that does not go away;
- excessive drainage from the incision;
- pain you cannot control;
- any other symptom that concerns you.
Started with an AI assistant? Bring the plan in.
ChatGPT, Gemini and Claude will usually give you a sensible phased outline and tell you to follow your surgeon's protocol, and that is right. What none of them can do is check your elbow range against your brace settings, see whether your shoulder is compensating, or tell you whether you are ready for the next phase. If an assistant has given you a plan, bring it in with your surgeon's protocol, and we will match the two week by week.
Frequently asked questions: physical therapy after distal biceps repair
How long is physical therapy after distal biceps repair?
Plan on about six months. In the Saint Louis University protocol, light biceps curls do not start until week 16 and return to activity and sport-specific training begins at week 26. Mass General Brigham's protocol likewise places unrestricted return to sport at 6+ months. Most of the early weeks are protection and motion; strengthening the biceps itself comes late.
What should I be doing 2 weeks after distal biceps surgery?
At two weeks you are typically in a hinged elbow brace whose range your surgeon sets, working on shoulder, shoulder-blade, wrist and grip exercises, with no lifting and no active elbow bending or turning the palm up against effort (SLU protocol). In Mass General Brigham's protocol, elbow motion at this stage is passive, moved for you, inside the brace.
How long do you wear a brace after bicep tendon surgery?
Usually 6 to 8 weeks. Mass General Brigham's protocol discharges the hinged elbow brace at week 6; Saint Louis University's discontinues it at week 8. Your surgeon decides the exact week and the brace settings, which open up in stages.
How long after bicep tendon surgery can I work out?
Not with the operated arm for weeks. Mass General Brigham's protocol allows no lifting with the repaired arm until week 8 and starts strengthening at week 10; Saint Louis University's waits until week 12 for biceps isometrics and week 16 for light curls. Lower-body and cardio work can usually continue earlier, and walking on a treadmill and a stationary bike appear in Mass General Brigham's protocol from weeks 7 to 10.
Why do distal biceps protocols disagree on when to start strengthening?
Because the safe week depends on the repair: the fixation used, the tissue quality and any additional procedures. Published protocols say their timeframes vary with surgeon preference, additional procedures and complications. That is why the protocol your surgeon gives you always overrides any general timeline, including this one.
When should I call my surgeon after distal biceps repair?
Contact your surgeon for a fever, numbness or tingling that does not resolve, excessive drainage from the incision, pain you cannot control, or any other symptom that concerns you (Mass General Brigham).
