The headache that is actually a neck problem
A large share of the headaches we see were never migraines. They are cervicogenic — referred from joints, discs and muscles in the upper neck — and they get treated for years as something else because nobody examined the neck.
The tell is usually in the pattern: the pain starts at the base of the skull and wraps forward, it is one-sided and stays on that side, it worsens with a long stretch at a desk or after sleeping awkwardly, and it can often be reproduced by pressing on the right spot in the neck. Migraine medication does little. That last detail is the one people mention most.
Patients come to us for this from across Ridgewood — the downtown and Van Neste Square area, the Route 17 corridor, and the commuter households near the train station where the combination of desk work and a long sit is a daily reality.
What we examine, and why the distinction matters
Cervicogenic headache
Referred from the upper cervical joints. Responds to manual treatment of those joints plus deep neck flexor retraining — not to rest, and not to painkillers.
Tension-type headache
Band-like, both sides, tied to sustained posture and stress. Overlaps with the above and often improves with the same work.
Cervical radiculopathy
A pinched nerve in the neck sending pain, pins and needles or weakness down the arm. Different problem, different treatment — see cervical radiculopathy.
Dizziness with neck pain
Worth flagging, because it sits between two things we treat in depth. Neck-related dizziness and true vestibular vertigo can feel similar and are managed very differently — we can test for both in one visit, which most clinics cannot. See vertigo and BPPV.
Genuine migraine
If that is what it is, we will say so and tell you this is not our tool. We are not going to sell you a course of neck treatment for a neurological condition.
Why one-on-one changes the outcome here
Neck treatment is hands-on and precise. Finding the segment that reproduces your headache, treating it, and then retraining the deep neck muscles that hold the correction is skilled manual work — it does not survive being handed to an aide with an exercise sheet.
