The question most Parsippany patients actually arrive with
Not “can you fix my knee” — it is “do I really need the replacement?”
Someone has looked at an X-ray, used the words bone-on-bone, and floated a joint replacement. That is the conversation we have most often with patients coming east from Lake Hiawatha, Mount Tabor, Lake Parsippany and the office parks along I-287 and Route 10.
Here is the honest position. Some joints do need replacing, and we will tell you plainly when we think yours is one of them. But the X-ray is a poor predictor on its own — plenty of people have dramatic imaging and little pain, and plenty have mild imaging and a great deal of it. What actually determines how a knee or hip feels is the strength, mechanics and load tolerance around it, and that is the part nobody has usually tested before surgery gets raised.
What a proper non-surgical attempt looks like
“Try physical therapy first” is standard advice that is very often delivered badly — a sheet of exercises, a few sessions with an aide, no measurable progression. If that is the trial you had, you have not really had one.
We measure before we prescribe
Strength, range, gait and the specific movements that provoke your pain get tested at the first visit so there is a baseline to argue with later.
Load is the treatment
Arthritic joints generally do better with progressively loaded strength work than with rest. The skill is in dosing it so the joint tolerates it, which is where one-on-one time actually matters.
We treat the joints either side
Hip weakness drives a lot of knee pain, and ankle stiffness drives a lot of hip pain. Treating only the joint that hurts is the most common reason a course of therapy fails.
You get a decision point, not an open-ended plan
We agree up front what improvement should look like and by when. If we get there, you have avoided an operation. If we do not, you go into that consultation stronger, which measurably helps recovery either way — see our pre-surgical physical therapy page.
Where shockwave fits — and where it does not
Shockwave therapy does not treat arthritis. It treats the tendon problems that often sit alongside it — gluteal tendinopathy and trochanteric bursitis at the hip, patellar tendinopathy at the knee — and those are frequently the actual source of the pain someone has been told is “just arthritis.” If that is what we find, it is worth knowing.
How we know the treatment is working before you pay for it
Shockwave only helps when the target tissue actually responds to the acoustic waves. Most places offering it locally — frequently a chiropractic office — sell a block of six or twelve sessions up front, before anyone knows whether your tissue will engage at all.
We check first. During the first application, Dr. Letizia looks for a specific biofeedback response at the treatment site: a clear sign the tissue is reacting the way it has to for shockwave to accomplish anything. If that response is not there, we stop and you are not charged for the treatment — you get an honest explanation of what is actually driving your pain instead. If it is there, we know we are on the right structure and can give you a realistic session estimate.
We do not know of another clinic in the area that screens this way or waives the charge when the response is not there. It is a screening step, not a promise of a specific outcome — be skeptical of anyone who tells you otherwise.
