Are You Struggling with These Scoliosis Problems?
Chronic Back Pain and Muscle Fatigue
Constant aching in your back that gets worse as the day goes on. Your muscles fatigue quickly because they're working overtime to support your curved spine. You struggle to sit or stand comfortably for extended periods, affecting work and daily activities.
Worried About Curve Progression
You or your child's curve has been increasing on X-rays. Your doctor is monitoring it closely and mentioned bracing or possibly surgery if it continues to progress. You want to do everything possible to prevent the curve from getting worse.
Postural Imbalance and Appearance Concerns
One shoulder sits higher than the other, your clothes don't hang right, or your rib cage is more prominent on one side. You're self-conscious about your appearance and how the curve affects your posture. You avoid certain clothes or activities because of how your back looks.
Limited Activities and Exercise
You've been told to avoid certain activities or exercises because of scoliosis. You're not sure what's safe and what might make your condition worse. You want to stay active but don't know how to exercise properly with a curved spine.

What Is Scoliosis?
Scoliosis is a lateral curvature of the spine that measures 10 degrees or more on X-ray. It can appear as an S-shaped or C-shaped curve, and it often includes a rotational component that affects rib position and trunk symmetry. While many people think of scoliosis as a childhood diagnosis, it affects adults just as frequently, sometimes as a continuation of adolescent curves and sometimes as a new development related to spinal degeneration.

At Spectrum Therapeutics of NJ in Wayne, Dr. Rob Letizia evaluates and treats scoliosis in both adolescents and adults using manual therapy, targeted exercise, and postural retraining.
Types of Scoliosis
- Adolescent idiopathic scoliosis (AIS): The most common type, typically diagnosed between ages 10 and 18. The cause is unknown, but it tends to run in families.
- Degenerative scoliosis: Develops in adulthood as spinal discs and joints wear down unevenly, often after age 50. This type frequently causes pain and stiffness.
- Adult idiopathic scoliosis: A progression of a curve that began in adolescence. Some adults do not realize they have scoliosis until back pain or postural changes bring them in for evaluation.
Can Physical Therapy Help Scoliosis?
Yes. Physical therapy cannot straighten a structural curve, but it can reduce pain, improve function, slow curve progression, and help you move with less restriction. For many people with scoliosis, therapy is the most practical and effective form of long-term management.
Treatment focuses on three things: reducing the muscular imbalances that develop around the curve, improving spinal mobility in stiff segments, and training the body to hold a more neutral posture throughout the day. In adolescents, a strong therapy program can complement bracing and potentially reduce the likelihood of surgical intervention.
Dr. Letizia uses a combination of orthopedic manual therapy and scoliosis-specific exercise approaches. Manual therapy addresses joint restrictions and soft tissue tightness directly. Therapeutic exercise builds the strength and motor control needed to support a more balanced spine. This includes elements drawn from the Schroth method, which uses specific breathing techniques and corrective postures to address the three-dimensional nature of scoliotic curves.
My scoliosis was getting worse every year and causing constant back pain. Dr. Rob designed a specific exercise program that reduced my pain and stabilized my curve.
What Exercises Help Scoliosis?
Effective scoliosis exercises are not generic core workouts. They are curve-specific, meaning the exercises are designed around the location and direction of your particular curve. Common components include:
- Asymmetric strengthening: Targeting the muscles on the concave side of the curve, which tend to be overactive and shortened, while lengthening the convex side
- Rotational breathing: Used in Schroth-based programs to expand the collapsed side of the rib cage and derotate the trunk
- Postural awareness drills: Retraining how you sit, stand, and move so that daily habits reinforce correction rather than reinforce the curve
- Core stabilization: Building deep trunk control to support the spine in a corrected position
- Flexibility work: Stretching tight structures on the concavity of the curve
These exercises are taught during sessions and assigned as a home program. Consistency with your home exercises is one of the biggest factors in long-term results.

Does Scoliosis Get Worse with Age?
It can. Curves over 30 degrees in skeletally mature adults tend to progress slowly over time, roughly 0.5 to 1 degree per year. Degenerative scoliosis can progress more quickly as disc and joint changes accelerate, especially after age 60. This is why ongoing management matters.
The good news is that progression is not inevitable, and even when a curve does increase slightly, keeping the surrounding muscles strong and the spine mobile can prevent the pain and functional decline that often accompanies progression. Patients throughout Wayne, Butler, Riverdale, and Passaic County come to Spectrum Therapeutics specifically to stay ahead of their scoliosis rather than reacting to it after problems develop.
When Should You Consider PT vs. Bracing vs. Surgery?
For adolescents with moderate curves (20 to 40 degrees), bracing is the standard medical recommendation to prevent progression during growth. Physical therapy works alongside bracing to maintain mobility and build strength that the brace alone cannot provide.
For adults with scoliosis-related pain, physical therapy is almost always the right first step. Surgery for scoliosis is a major undertaking, typically involving spinal fusion across multiple levels, and is generally reserved for curves over 50 degrees with significant functional impairment or neurological symptoms that have not responded to conservative care.
Most scoliosis patients never need surgery. A well-structured physical therapy program can keep you active, reduce pain, and maintain quality of life for years.
My daughter was told she might need surgery for her scoliosis. Dr. Rob started her on a specialized program. Her curve has not progressed in 18 months.

Frequently Asked Questions
Can physical therapy straighten my scoliosis curve?
Physical therapy cannot fully straighten a structural scoliosis curve. However, it can reduce pain, improve posture and alignment, strengthen muscles to support your spine better, and potentially slow or stop curve progression in some cases. Our goal is to help you function at your best and maintain quality of life with your scoliosis, not to eliminate the curve completely.
What types of scoliosis do you treat?
We treat all types of scoliosis including: Adolescent idiopathic scoliosis in teens and young adults, adult scoliosis in people diagnosed as adults or continuing from adolescence, degenerative scoliosis developing later in life due to arthritis and disc degeneration, and post-surgical scoliosis rehabilitation after fusion or other corrective procedures. Each type requires a customized treatment approach.
Is exercise safe with scoliosis?
Yes! Exercise is not only safe but essential for managing scoliosis. However, certain exercises may need to be modified based on your curve pattern and severity. We teach you which exercises are most beneficial, which to avoid, and how to perform activities safely. Many people with scoliosis participate in sports, fitness activities, and even high-level athletics with proper guidance.
How long does scoliosis treatment take?
Initial treatment typically involves 8-12 weeks of therapy to learn your exercise program and establish proper techniques. After that, many patients transition to an independent home program with periodic check-ins. For adolescents with progressing curves, ongoing therapy may be recommended during growth years. Adults with chronic pain may benefit from maintenance therapy on an as-needed basis.
